RESUMO

Background: Non-suicidal self-injury (NSSI) has become a significant public health issue, especially prevalent among adolescents. The complexity and multifactorial nature of NSSI necessitate a comprehensive understanding of its underlying causal factors. This research leverages the causal discovery methodology to explore these causal associations in children. Methods: An observational dataset was scrutinized using the causal discovery method, particularly employing the constraint-based approach. By integrating machine learning and causal inference techniques, the study aimed to determine direct causal relationships associated with NSSI. The robustness of the causal relationships was evaluated using three methods to construct and validate it: the PC (Peter and Clark) method, Fast Causal Inference (FCI) method, and the GAE (Graphical Autoencoder) method. Results: Analysis identified nine nodes with direct causal relationships to NSSI, including life satisfaction, depression, family dysfunction, sugary beverage consumption, PYD (positive youth development), internet addiction, COVID-19 related PTSD, academic anxiety, and sleep duration. Four principal causal pathways were identified, highlighting the roles of lockdown-induced lifestyle changes, screen time, positive adolescent development, and family dynamics in influencing NSSI risk. Conclusions: An in-depth analysis of the factors leading to Non-Suicidal Self-Injury (NSSI), highlighting the intricate connections among individual, family, and pandemic-related influences. The results, derived from computational causal analysis, underscore the critical need for targeted interventions that tackle these diverse causative factors.

RESUMO

BACKGROUND: While national adoption of universal HIV treatment guidelines has led to improved, timely uptake of antiretroviral therapy (ART), longer-term care outcomes are understudied. There is little data from real-world service delivery settings on patient attrition, viral load (VL) monitoring, and viral suppression (VS) at 24 and 36 months after HIV treatment initiation. METHODS AND FINDINGS: For this retrospective cohort analysis, we used observational data from 25 countries in the International epidemiology Databases to Evaluate AIDS (IeDEA) consortium's Asia-Pacific, Central Africa, East Africa, Central/South America, and North America regions for patients who were ART naïve and aged ≥15 years at care enrollment between 24 months before and 12 months after national adoption of universal treatment guidelines, occurring 2012 to 2018. We estimated crude cumulative incidence of loss-to-clinic (CI-LTC) at 12, 24, and 36 months after enrollment among patients enrolling in care before and after guideline adoption using competing risks regression. Guideline change-associated hazard ratios of LTC at each time point after enrollment were estimated via cause-specific Cox proportional hazards regression models. Modified Poisson regression was used to estimate relative risks of retention, VL monitoring, and VS at 12, 24, and 36 months after ART initiation. There were 66,963 patients enrolling in HIV care at 109 clinics with ≥12 months of follow-up time after enrollment (46,484 [69.4%] enrolling before guideline adoption and 20,479 [30.6%] enrolling afterwards). More than half (54.9%) were females, and median age was 34 years (interquartile range [IQR]: 27 to 43). Mean follow-up time was 51 months (standard deviation: 17 months; range: 12, 110 months). Among patients enrolling before guideline adoption, crude CI-LTC was 23.8% (95% confidence interval [95% CI] 23.4, 24.2) at 12 months, 31.0% (95% CI [30.6, 31.5]) at 24 months, and 37.2% (95% [CI 36.8, 37.7]) at 36 months after enrollment. Adjusting for sex, age group, enrollment CD4, clinic location and type, and country income level, enrolling in care and initiating ART after guideline adoption was associated with increased hazard of LTC at 12 months (adjusted hazard ratio [aHR] 1.25 [95% CI 1.08, 1.44]; p = 0.003); 24 months (aHR 1.38 [95% CI 1.19, 1.59]; p < .001); and 36 months (aHR 1.34 [95% CI 1.18, 1.53], p < .001) compared with enrollment before guideline adoption, with no before-after differences among patients with no record of ART initiation by end of follow-up. Among patients retained after ART initiation, VL monitoring was low, with marginal improvements associated with guideline adoption only at 12 months after ART initiation. Among those with VL monitoring, VS was high at each time point among patients enrolling before guideline adoption (86.0% to 88.8%) and afterwards (86.2% to 90.3%), with no substantive difference associated with guideline adoption. Study limitations include lags in and potential underascertainment of care outcomes in real-world service delivery data and potential lack of generalizability beyond IeDEA sites and regions included in this analysis. CONCLUSIONS: In this study, adoption of universal HIV treatment guidelines was associated with lower retention after ART initiation out to 36 months of follow-up, with little change in VL monitoring or VS among retained patients. Monitoring long-term HIV care outcomes remains critical to identify and address causes of attrition and gaps in HIV care quality.

RESUMO

BACKGROUND: This study aimed to evaluate the immunogenicity and safety of different types of poliovirus vaccines. METHODS: A randomized, blinded, single-center, parallel-controlled design was employed, and 360 infants aged ≥ 2 months were selected as study subjects. They were randomly assigned to bOPV group (oral Sabin vaccine) and sIPV group (Sabin strain inactivated polio vaccine), with 180 infants in each group. Adverse reaction events in the vaccinated subjects were recorded. The micro-neutralization test using cell culture was conducted to determine the geometric mean titer (GMT) of neutralizing antibodies against poliovirus types I, II, and III in different groups, and the seroconversion rates were calculated. RESULTS: Both groups exhibited a 100% seropositivity rate after booster immunization. The titers of neutralizing antibodies for the three types were predominantly distributed within the range of 1:128 to 1:512. The fold increase of type I antibodies differed markedly between the two groups (P < 0.05). Moreover, the fold increase of type II and type III antibodies for poliovirus differed slightly between the two groups (P > 0.05). The fourfold increase rate in sIPV group was drastically superior to that in bOPV group (P < 0.05). When comparing the post-immunization GMT levels of type I antibodies in individuals who completed the full course of spinal muscular atrophy vaccination, bOPV group showed greatly inferior levels to sIPV group (P < 0.05). For type II and type III antibodies, individuals in bOPV group demonstrated drastically superior post-immunization GMT levels to those in sIPV group (P < 0.05). The incidence of adverse reactions between the bOPV and sIPV groups differed slightly (P > 0.05). CONCLUSION: These findings indicated that both the oral vaccine and inactivated vaccine had good safety and immunogenicity in infants aged ≥ 2 months. The sIPV group generated higher levels of neutralizing antibodies in serum, particularly evident in the post-immunization GMT levels for types II and III.

RESUMO

INTRODUCTION: Neglected diseases are a significant global health challenge. Encouraging the development of therapeutics and vaccines for these diseases would address an important unmet medical need. We propose a priority review voucher programme for the European Union (EU). The developer of a drug or vaccine for a neglected disease would receive a voucher for accelerated assessment of a different product at the European Medicines Agency (EMA). METHODS: This study uses retrospective observational data to estimate the potential commercial value of the proposed voucher programme using a five-step approach: (1) estimating the time saved in the EMA accelerated regulatory review; (2) gauging time reductions in accelerated pricing and reimbursement decisions by EU member states; (3) selecting 10 high-revenue products launched between 2015 and 2020 representing typical voucher users; (4) analysing IQVIA MIDAS sales data for the selected products and (5) calculating the net present value (NPV) of the voucher based on the 10 products. RESULTS: The accelerated EMA review would reduce regulatory time by an average of 182 days. Additionally, products could save more than a year in many member states through an expedited 120-day pricing and reimbursement review. The estimated NPV of regulatory acceleration by two quarters would be €100 million. In addition, if France, Italy and Spain reviewed pricing and reimbursement in only 120 days, then the value would double. CONCLUSION: An EU voucher estimated at more than €100 million, coupled with a US$100 million counterpart, offers a meaningful incentive for novel product development. However, the voucher programme should be part of a comprehensive strategy for tackling neglected diseases, rather than a standalone solution.

This paper reflects on the concepts of transference and countertransference in articulation with the clinical dimension of the observer's place in therapeutic groups. After a brief historical overview of these concepts in psychoanalysis, the notion of transference is discussed as a link that can manifest itself via bodily and sensorially. Countertransference, in turn, will be analyzed based on intersubjectivity. These aspects will be illustrated by two clinical vignettes

RESUMO

OBJECTIVES: To compare mortality rates in bipolar disorder with common causes of mortality. METHODS: Observational data from the Prechter Longitudinal Study of Bipolar Disorder (PLS-BD) of 1128 participants including 281 controls was analyzed using logistical regression to quantify mortality rates in comparison with common comorbidities and causes of death. Outcome and treatment measures, including ASRM, GAD-7, PHQ-9 and medication use were used to stratify those with bipolar disorder (BD) that are alive or deceased. A larger cohort of 10,735 existing BD patients with 7,826 controls (no psychiatric diagnosis) from the University of Michigan Health (U-M Health) clinics was used as replication, observational secondary data analysis. RESULTS: The mortality rates are significantly different between those with BD and controls in both PLS-BD and U-M Health. Those with BD and are deceased have a higher percentage of elevated depression measures but show no difference in mania or anxiety measures nor medication use patterns. In both cohorts, a diagnosis of BD increases the odds of mortality greater than history of smoking or being older than ≥ 60-years of age. CONCLUSION: BD was found to increase odds of mortality significantly and beyond that of a history of smoking. This finding was replicated in an independent sample.

RESUMO

BACKGROUND: The System of Observing Play and Leisure Activity in Youth (SOPLAY) has assisted in providing valid and reliable data of youth physical activity and characteristics specific to environmental contexts. The review aimed to examine empirical research that employed the SOPLAY instrument to measure physical activity in leisure-based activity environments in North American countries. METHODS: The review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. A systematic search was conducted with an exhaustive process using 10 electronic databases to locate peer-reviewed studies implementing SOPLAY that were published between 2000 and 2021. RESULTS: A total of 60 studies were included in the review. Most studies (n = 35) reported physical activity findings related to contextual characteristics measured using SOPLAY. Interestingly, a few of the studies (n = 8) found the provision of equipment and supervision, particularly adult supervision, significantly increased child physical activity observed. CONCLUSIONS: This review provides information relative to the group-level physical activity observed across multiple contexts (ie, playgrounds, parks, recreation centers) using a validated direct observation instrument.

RESUMO

BACKGROUND: Using infrared counters is a promising unobtrusive method of assessing footfall in urban parks. However, infrared counters are susceptible to reliability and validity issues, and there is limited guidance for their use. The aims of this study were to (1) determine how many weeks of automated active infrared count data would provide behaviourally stable estimates of urban park footfall for each meteorological season, and (2) determine the validity of automated active infrared count estimates of footfall in comparison to direct manual observation counts. METHODS: Three automated active infrared counters collected daily footfall counts for 365 days on three footpaths in an urban park within Northampton, England, between May 2021 - May 2022. Intraclass correlation coefficients were used to compare the behavioural stability of abbreviated data collection schedules with total median footfall within each meteorological season (Spring, Summer, Autumn, Winter). Public holidays, events, and extreme outliers were removed. Ten one-hour manual observations were conducted at the site of an infrared counter to determine the validity of the infrared counter. RESULTS: At least four-weeks (28 days) of infrared counts are required to provide 'good' to 'excellent' (Intraclass correlation > 0.75, > 0.9, respectively) estimates of median daily footfall per meteorological season in an urban park. Infrared counters had, on average, -4.65 counts per hour (95% LoA -12.4, 3.14; Mean absolute percentage error 13.7%) lower counts compared to manual observation counts during one-hour observation periods (23.2 ± 15.6, 27.9 ± 18.9 counts per hour, respectively). Infrared counts explained 98% of the variance in manual observation counts. The number of groups during an observation period explained 78% of the variance in the difference between infrared and manual counts. CONCLUSIONS: Abbreviated data collection schedules can still obtain estimates of urban park footfall. Automated active infrared counts are strongly associated with manual counts; however, they tend to underestimate footfall, often due to people in groups. Methodological and practical recommendations are provided.

RESUMO

An integrated intersectoral care model promises to meet complex needs to promote early child development and address health determinants and inequities. Nevertheless, there is a lack of understanding of actors' interactions in producing intersectoral collaboration networks. The present study aimed to analyze the intersectoral collaboration in the social protection network involved in promoting early child growth and development in Brazilian municipalities. Underpinned by the tenets of actor-network theory, a case study was conducted with data produced from an educational intervention, entitled "Projeto Nascente." Through document analysis (ecomaps), participant observation (in Projeto Nascente seminars), and interviews (with municipal management representatives), our study explored and captured links among actors; controversies and resolution mechanisms; the presence of mediators and intermediaries; and an alignment of actors, resources, and support. The qualitative analysis of these materials identified three main themes: (1) agency fragility for intersectoral collaboration, (2) attempt to form networks, and (3) incorporation of fields of possibilities. Our findings revealed that intersectoral collaboration for promoting child growth and development is virtually non-existent or fragile, and local potential is missed or underused. These results emphasized the scarcity of action by mediators and intermediaries to promote enrollment processes to intersectoral collaboration. Likewise, existing controversies were not used as a mechanism for triggering changes. Our research supports the need to mobilize actors, resources, management, and communication tools that promote processes of interessement and enrollment in favor of intersectoral collaboration policies and practices for child development.

The physical characteristics of the environment can contribute to health promotion, especially the availa-bility and quality of public leisure spaces. The present study aims to analyze the distribution and quality of equipment for leisure physical activity in a Brazilian capital. Twenty-seven public spaces, including parks, squares and boulevards available within a radius of 1,000 meters of three centers of the Health Academy Program (PAS) in the city of Belo Horizonte were evaluated. In 2019, the equipment was audited through the application of the adapted version of the Physical Activity Resource Assessment (PARA), which evaluates the existence, quality, safety, cleanliness, and aesthetics of the equipment. Absolute and relative frequencies were calculated. Of the 27 sites evaluated, 22.2% were parks (n=6), 70.4% were squares (n = 19), and 7.4% were boulevards (n = 2). Volleyball courts and soccer fields were the most identified equipment in parks, present in all analyzed places, while outdoor gyms and exercise stations were the most common in squares (89.5%; 52.9%, respectively). As for boulevards, they only had volleyball courts/soccer fields. Regarding cleanliness and aesthetics, the highest proportions were graffiti (74.1%), scattered garbage/dirt (74.1%), tall grass (44.4%), and signs of vandalism and animal dirt (37.0%). The small diversity and poor quality of equipment, and the inadequate characteristics of cleanliness and aesthetics of the leisure spaces in which they are located, can prevent or hinder their use for the practice of physical activity, thus demonstrating the need for more expressive investments in these places

RESUMO

PURPOSE: To investigate the nutrition-related care needs of older patients in hospitals. METHODS: A qualitative multimethod study was performed. By purposive sampling, older patients were included for observations of the nutrition process and interviews from a perioperative unit, an internal Medicine unit, and one acute geriatric care ward of an acute care hospital. Preliminary findings were discussed, validated, and further explored in two focus group discussions with interprofessional nutritional healthcare experts. FINDINGS: Seventeen women and five men with a mean age of 86 years participated in patient observations and interviews. Eight experts with at least 10 years of professional experience who were employed in this setting for 3 years or more participated in interviews. Three themes of nutrition-related care needs were elaborated: The need to assess and address older patients' attitudes toward life, the need for nutrition-related education, and the need for food intake support. These care needs added to the development and validation of a new nursing diagnosis "risk for inadequate protein energy nutrition" and in consequence to develop a complex nursing intervention to optimize the nutrition of older patients. CONCLUSIONS: It is crucial to assess and understand the patients' attitudes toward life, which affect purposes of treatment, diet, and menu choices. IMPLICATIONS FOR NURSING PRACTICE: Implementation of assessing nutrition-related care needs of older inpatients is needed. Addressing these care needs within the Advanced Nursing Process can lead to appropriate nursing diagnoses, nursing outcomes, and interventions, which enhance person-centered care, patients' self-care abilities, and consequently patients' nutritional status.

Airborne mineral dust triggers ice formation in clouds and alters cloud microphysical properties by acting as ice-nucleating particles (INPs), potentially influencing weather and climate at regional and global scales. Anthropogenic pollution would modify natural mineral dust during the atmospheric transport process. However, the effects of anthropogenic pollution aging on the ice nucleation activity (INA) of mineral dust remain not well-understood. In this study, we investigated the immersion mode ice nucleation properties and particle chemical characterizations of collected size-resolved Asian dust samples (eight particle size classes ranging from 0.18 to 10.0 µm), and testified the chemical modification of aged dust particles via particle chemistry and morphology analyses including the mass concentrations of particulate matter, the water-soluble ion concentrations, the mental element concentrations, and single-particle morphology. The mass fraction of Ca2+ in element Ca and the mean relative mass proportions of supermicron Ca2+ increased by 67.0 % and 3.5-11.2 % in aged Asian dust particles, respectively, suggesting the occurrence of heterogeneous reactions. On the other hand, the total INP concentrations (total NINP) and total ice nucleation active site densities (total ns(T)) were consistent between aged and normal dust particles (0.62-1.18 times) without a statistically significant difference. And the NINP and ns(T) of chemically aged supermicron dust (1.0-10.0 µm) in each particle size class were nearly equal to or slightly higher than those of normal Asian dust, which were 0.70-2.45 times and 0.64-4.34 times at -18 °C, respectively. These results reveal that anthropogenic air pollution does not notably change the INP concentrations and does not impair the INA of Asian dust. Our work provides direct observational evidence and clarifies the non-suppression effect of anthropogenic air pollution on the INA of East Asian dust, advancing the understanding of the ice nucleation of airborne aged mineral dust.

ABSTRACT This study aimed to assess the effect of standardizing observation and rating criteria for the front crawl stroke technique on inter- and intra-rater reliability in order to propose an observation list. Two teachers and two competitive swimming coaches with different academic backgrounds were responsible for evaluating the front crawl stroke technique of 44 swimmers using video. Each swimmer completed a 25m course at a comfortable speed, having their displacement recorded in the lateral and frontal planes, both submerged and out of water, in the final 10 m of the pool. Throughout the recording, the swimmers did not breath laterally. Only the movement performed by the right arm was the raters' focus of observation and was based on a checklist. After the shooting, the raters participated in three stages: in the first stage, there was an intervention to standardize the observation and rating criteria; inter-rater agreement was tested in the second stage, while intra-rater agreement was obtained in the third. In the second stage of the study, the agreement ranged from small to low (k=0.08 and k=0.38). In the third stage, some raters showed poor and low agreement (k=-0.13 and k=0.35), while others showed agreement between reasonable and virtually perfect (k=0.64 and k=0.87). From the results presented, it was concluded that the intervention proposal did not have the desired effect of standardizing the observation and rating criteria among the raters.

Some mothers, even when joined by a partner, end up playing multiple roles alone, taking care of the household chores, their work lives and, precariously, of themselves. In order to discuss motherhood in a context of vulnerability and its implications in the establishing of the relationship between a mother and her children, based on a specific case, an psychoanalytic observational method inspired by the Bick Method of Observation was employed, being a qualitative research. The observations were carried out considering three tempos: weekly observation, on the same day and time; observation report, in an involved and descriptive way; and supervision seminar, to discuss the observations. It was made evident the helplessness experienced by this woman-mother, who, despite the presence of her husband and family, did not receive the necessary support. Her exhaustion, stemming from this helplessness, resulted in a loss of emotional control over her children. The value of the Bick Observation Method is highlighted, as it enables a sensitive outlook towards motherhood

This paper is concerned with evaluating the possibilities of the observational method applied to the analysis of problem-solving strategies in children with disabilities through structured tasks. The theoretical framework of reference for this contribution is represented by the theory of problem-solving, containing the main processes involved in problem management, which we apply to atypical cognitive and metacognitive development. Together with three different structured tasks (Tower of Hanoi, Tetris, and Building Blocks), we used two lists of behaviors to code problem-solving strategies adopted by children and/or adolescents with atypical development. The application of this type of observation with the use of lists of behaviors could be useful to create a link between qualitative and quantitative research to improve the knowledge of problem-solving strategies and metacognitive competencies in atypically developed children(AU)

Introduction: adolescent pregnancy refers to any gestation that occurs during adolescence and includes women up to 19 years of age. It bursts into the lives of adolescents at times when they do not reach physical or mental maturity, with nutritional deficiencies and illnesses. The causes are broad and complex, varying among different cultural and ethnic groups. For researchers, it is important to know the way of thinking of young women and mothers regarding teenage pregnancies, how they build meanings and meanings around early pregnancies, what factors intervene, how the young and adult population can be educated before this problem that bursts with his life project. Material and methods: the present investigation has a descriptive qualitative methodological approach, with a holistic historical-ethnographic vision, since it is intended to know the way of thinking of adolescents from the state of Guerrero in the high mountain region before a pregnancy and the possible causes of it. Results: the binomial reveals that the lack of economic resources, education, customs and traditions are the main causes of early pregnancy and school dropout in the High Mountain of Guerrero. Final considerations: the term EMIC shows disagreement with adolescent pregnancy, the inability of the participants to develop the role of upbringing and education, the physical and psychological immaturity of the adolescents to acquire the responsibility of an adult woman is reflected in ETIC, the life project is stagnated in the same pattern of the parents.
